Terms & Conditions
I hereby give my permission for my/my child’s participation in any and all activities prepared by PTC and any outside events.
I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Phoenix and all its respective officers, agents, and representatives from any and all liability for injuries to myself or said child, arising out of participating in Saturday workshops and other PTC events. All appropriate safety measures are in place and all Phoenix staff have enhanced DBS certificates.
I accept that I must give two months' paid notice in the event my child/student wishes to leave Academy, after one initial trial session.
WE REQUIRE TWO FULL MONTHS NOTICE IN WRITING SHOULD YOUR CHILD DECIDE TO LEAVE ANY PTC ACADEMY CLASSES. Once a student joins any of our classes, this contract (application/registration form) is ongoing & assumed to run on from term to term unless due notice is given as above. If a student leaves ANY OF THE ACADEMY CLASSES THEY SIGN UP TO without notice being given, then two more monthly fees must be paid in lieu of notice. Failure to pay any fees owed will be referred to our Solicitor & action taken to recoup funds as necessary. THESE CONDITIONS APPLY TO ALL PTC Academy STUDENTS.
I accept I must give one month's paid notice in the event I/my child wishes to leave other weekday sessions
I agree to pay all fees over 12 months on the first (1st) of each month, using the ClassForKids system.
I will regularly update all relevant personal and health information.
I will keep updated on class information by following the PTC Whatsapp and Facebook group, and ensure that I/my child arrives to all sessions and activities on time, in the correct uniform or clothing.
